  1. You have to be obese.

  2. You have to have one or more medical conditions related to your obesity, such as:

    · Type II Diabetes
    · Chronic Fatigue Syndrome
    · Arthritis
    · Hernia
    · High Triglycerides
    · Fibromyositis
    · Reactive Hypoglycemia
    · Polycystic Ovary Syndrome
    · Infertility
    · Sleep Apnea
    · Snoring

  3. A letter of referral written by your family physician or specialist stating that you have a medical condition that requires weight reduction. Click here to download a referral sample letter.

  4. A copy of your blood test from your last check up.

  7. The first appointment will last about 30 minutes. Please bring all your medications with you to show the doctor.
